This is a Request for Proposal for the services as stated in the attached Performance Work Statement. The offeror shall include in their proposal the company's taxpayer identification number, the company's DUNS number, the company's address, the company's point of contact and contact information, the firm fixed-price for all services in accordance with the solicitation instructions attached. Please note that this solicitation is 2-phase approach as stated in the solicitation instructions.
